Victim Notification Questionnaire Information

Thank you for responding to the victim notification letter that you received regarding this investigation. As indicated in the victim notification letter, you can obtain additional information about the investigation and prosecution at http://www.justice.gov/usao/ct/Press2012/20121211-1.html.

Why were you contacted?

We are contacting you because your name and address were found during an investigation into a credit card fraud and identity theft scheme. In addition, the credit or debit card number provided in the letter you received was found in the possession of the defendants on the date indicated in the letter.

To protect your right to restitution and to provide information to the court about any loss that you may have experienced, we ask that you please fill out a brief victim impact form.

Your questionnaire is confidential. The FBI will need your contact information so we can keep you informed of potential progress in any federal investigation that may develop as deemed appropriate, as well as provide you updated information regarding your rights in this matter. If it is determined you are not a potential victim, you will be removed from the FBI’s victim notification system. Please submit your questionnaire by January 31, 2013.

The victim notification letter you received provided a redacted credit or debit card account number that was recovered during this investigation. Was that a valid credit or debit card account number issued to you on or about the date indicated?  


It is important for the court to know if there were any fraudulent charges on that credit or debit card account on or around the date indicated in the letter, regardless of whether the charges were reversed or whether you were reimbursed.
