Questionnaire for End Users of Human Anatomical Material

The Detroit Division of the FBI is conducting an investigation into the activities of the following human body donation companies/programs:

- Biological Resource Center (Phoenix, Arizona)
- Biological Resource Center of Illinois (Rosemont/Schiller Park, Illinois)
- International Biological, Inc. (Detroit, Michigan)
- Arthroscopy Association of North America (Schiller Park, Illinois)
- Anatomical Service, Inc. (Schiller Park, Illinois)

If you or your organization acquired human anatomical material from any of these companies, we ask that you complete the below questionnaire to assist us in obtaining more information that will aid in the investigation.

1. From which company(s) did you/your organization purchase or lease human anatomical material?   Select all that apply.
2. Did you/your organization receive any literature via mail, e-mail, or fax about the company and its products or services?   Select all that apply.
3. Are/were there any contracts or purchase agreements between you/your organization and the company?

5. Did any contract, purchase agreement, or other documentation claim that all the human anatomical material had been properly and legally consented?  


6. Did the provider make specific claims about negative serology results?  


7. Did the provider make any claims of general non-infectiousness with regard to the human anatomical material?
