If you have been directed to this questionnaire via an FBI communication, it is because your business has been identified as a potential victim in a federal investigation. The FBI and Las Vegas Metropolitan Police Department are investigating a cyber intrusion into ATMs which allowed unauthorized people to withdraw money. The FBI has identified your business as a potential victim and is requesting your assistance with this investigation. As such, please answer the questions below to the best of your ability. Based on your responses, you may be contacted by the FBI and asked to provide additional information.
