If you have been directed to this questionnaire via an FBI victim notification letter, it is because you have been identified as having been a potential investor in Swiftarc Capital, Swiftarc Ventures, or with Siddharth Jawahar. To assist in the investigation, please answer the questions below to the best of your ability. It may be that you have already given this information to an FBI agent, but please provide it again on this questionnaire. Based on your responses, you may be contacted by the FBI or the United States Attorney’s Office - Eastern District of Missouri and asked to provide additional information.

Participation is voluntary; however, your responses may be helpful to the ongoing investigation and help determine if you were the victim of a crime. Based on the responses, the FBI may contact you to request additional information. Thank you for your patience as we review submissions.
