The FBI’s Buffalo Division is returning medical records to patients of Dr. Muhammad Cheema, who was charged with one count of a violation of Title 18, United States Code 1347 (Health Care Fraud) in 2018. The medical records are of Dr. Muhammad Cheema’s patients while he was practicing out of Upstate Psychiatry and Fairport Psychiatry between 2013-2017. Upstate Psychiatry and Fairport Psychiatry were both located at 1387 Fairport Rd, Fairport, New York.

If you and/or your minor dependent(s) were patients of Dr. Muhammad Cheema and would like their medical records returned, please fill out this short form.
